Zeiglers wunderbare Welt des Wohnzimmers

Schauen Sie ruhig mal hin. Tiefer. Noch tiefer. Ja genau, auf die Schuhe des Studiogastes. Wie - der hat keine an? Das war Ihnen noch gar nicht aufgefallen? Dabei ist es in diesem TV-Studio Gesetz. Die Gäste ziehen die Schuhe aus. Alle.

Bevacizumab - Wirklich effektiv?

The key finding of this study was that, in the overall population, there was a statistically significant benefit from the addition of bevacizumab to fluorouracil-based chemotherapy. However, this benefit was felt to be “modest” and “no more than marginally effective.” Moreover, subset analyses found little to no benefit in patients treated with oxaliplatin. These findings, or at least the conclusions, seem to depart from the majority of previously published data from randomized controlled clinical trials (RCTs), registries, and meta-analyses, including those that have focused on elderly patients.213 How can this discordance be reconciled? Aspects of Anatomy and War on Error

Aspects of Anatomy is a substantial part of a full anatomy course in your pocket. Peter describes the body's structures clearly and concisely in short videos, and you can then test yourself with multiple-choice questions. Mehr

The causes of medical error are many, but rather than point the finger of blame we need to understand why they happen, identify those that can be avoided, and develop ways to design them out of the system.

Intimate moments: the lives of Afghan women

She had the face of a girl, but the eyes of a middle-aged woman. Sitting in white pyjamas on a hospital bed in Fayzabad, the capital of the Afghan province of Badakhshan, 25-year-old Rahima Aziz told me how her baby died. Like so many other women in Afghanistan, she had tried to give birth at home in her remote village, with no clinic or midwife nearby. After 8 hours in a borrowed car on unpaved mountain roads, she finally reached the provincial hospital in Fayzabad but her baby had already died. “I carried the baby for 9 months”, she said, “And now it's gone. It's the third time this happens.” Mehr

At what age are surgeons safest?

At what age are surgeons safest? In France, according to Antoine Duclos and colleagues, it’s between 35 and 50 years old (doi:10.1136/bmj.d8041). The authors looked prospectively at thyroid operations performed in five high volume centres and found an increased risk of permanent complications when operations were done by less experienced surgeons and those in practice for more than 20 years. This finding has a certain face validity, but the authors recommend caution in interpreting their results. They looked at only one type of operation and used a cross sectional study design. Future research might follow a cohort of surgeons to see how performance changes during a surgeon’s career, they say. Supervision in the early years is an obvious response, but what should surgeons do when they reach 50?

The art of medicine: Music, medicine, and embodiment

I strongly suspect that the consolations of music are more apparent, and to overwhelmingly more people, than are those of any other form of art. This is refl ected by the way in daily life so many people voluntarily and habitually access music rather than any other art form. Then why is music so overwhelmingly popular? Mehr

The power of positive thinking...

The campaign for sainthood for Mother Mary MacKillop began in 1927. In the late 1800s, the Australian nun was a diligent campaigner for educating the poor and the cofounder of a new religious order, but the prayers of two cancer survivors after her death are what ensured Mary’s canonisation later this year. In 2009, Mother Mary was credited with her second miracle after a woman in New South Wales was pronounced clear of inoperable lung cancer and a secondary brain tumour. The woman had no standard treatment but wore a relic of Mary’s clothing and prayed to her daily. The first miracle attributed to Mother Mary was the similar healing of a woman with leukaemia Mehr

…And the only side-effects are good ones

...For example, in the EPIC study,  patients who never smoked, had a body-mass index less than 30 kg/m², had at least 30 min a day of physical activity, and adhered to healthy dietary principles (high intake of fruits, vegetables, and whole-grain bread and low meat consumption) had a 78% lower overall risk of developing a chronic disease. This included a 93% reduced risk of diabetes, an 81% lower risk of myocardial infarction, a 50% reduction in risk of stroke, and a 36% overall reduction in risk of cancer, compared with participants without these healthy factors. Galen's patients

Today, Galen of Pergamon is best known as the most influential exponent of the ancient world’s cumbersome medical doctrines—especially humoral theory—and for many subtle anatomical discoveries, as well as mistakes about human anatomy and physiology. Galen is antiquity’s most prolifi c author in Greek, and his works formed the basis of medical education in the Byzantine empire and in Europe for many centuries. But Galen saw himself mainly as a medical practitioner and not as an anatomist, theorist, or researcher. He treated patients in the Roman empire, and mainly in the city of Rome, for many decades—from about 162 CE until his death some time after 203—and his works are today our most immediate insight into how medicine was practised by
the ancient world’s most highly trained doctors. Mehr

Sport gegen Krebs

Objective To systematically evaluate the effects of physical activity in adult patients after completion of main treatment related to cancer.
Design Meta-analysis of randomised controlled trials with data extraction and quality assessment performed independently by two researchers.
Data sources Pubmed, CINAHL, and Google Scholar from the earliest possible year to September 2011. References from meta-analyses and reviews.
Study selection Randomised controlled trials that assessed the effects of physical activity in adults who had completed their main cancer treatment, except hormonal treatment.
Results There were 34 randomised controlled trials, of which 22 (65%) focused on patients with breast cancer, and 48 outcomes in our meta-analysis. Twenty two studies assessed aerobic exercise, and four also included resistance or strength training. The median duration of physical activity was 13 weeks (range 3-60 weeks). Most control groups were considered sedentary or were assigned no exercise. Based on studies on patients with breast cancer, physical activity was associated with improvements in insulin-like growth factor-I, bench press, leg press, fatigue, depression, and quality of life. When we combined studies on different types of cancer, we found significant improvements in body mass index (BMI), body weight, peak oxygen consumption, peak power output, distance walked in six minutes, right handgrip strength, and quality of life. Sources of study heterogeneity included age, study quality, study size, and type and duration of physical activity. Publication bias did not alter our conclusions.
Conclusions Physical activity has positive effects on physiology, body composition, physical functions, psychological outcomes, and quality of life in patients after treatment for breast cancer. When patients with cancer other than breast cancer were also included, physical activity was associated with reduced BMI and body weight, increased peak oxygen consumption and peak power output, and improved quality of life.
